The Treatment Process
Before we can begin working on achieving your goals, there are a few forms that need to be filled out by you and your doctor/physician.
In order for us to get started, complete the following steps with the appropriate forms attached below:
1) Sign and return the Therapy Management Agreement along with a copy of your driver’s license, either via email or fax (772-463-9565) (you can take a picture of it with your phone and send it to email@example.com).
2) Complete the Health History Intake Form and email (firstname.lastname@example.org) or fax (772-463-9565) back along with signed Therapy Management Agreement.
3) Have a physical exam completed at your PCP’s office, local urgent care, minor emergency center or minute clinic. The practitioner must fill out and sign OUR physical examination form. Email (email@example.com) or fax (772-463-9565) the form back to our office.
4) Once all previous steps are completed and all documentation has been received, please give us 24-48 hours to review your file before reaching out to us.
